Payment Terms:
A credit card is needed to reserve your seat in a course,
even if you are paying by cheque. Please do not
bring the cheque with you to class; sent it to the
order of Source 1 Consultants, 4571 Cumberland Avenue,
Montreal, QC H4B 2L5. If your cheque is not received
ten business days prior to the course, your credit card
will be charged.
DO NOT PUT CREDIT CARD INFORMATION ON THIS FORM. A CONFIRMATION
OF YOUR REGISTRATION WILL BE SENT TO YOU BY EMAIL OR
FAX. THIS CONFIRMATION WILL HAVE INSTRUCTIONS ON
HOW TO COMPLETE YOUR PAYMENT.
Note: The minimum class size is 3-4 participants. Should
either session not meet the minimum registration the
class could be canceled. Cancellations by a participant
are permitted without penalty 11 or more business days
prior to the scheduled onsite training date. Cancellations
made within 10 business days of the workshop start date
will be invoiced at 100% of the training fee. Rescheduling
is permitted without penalty 11 or more business days
prior to the scheduled onsite training date. We can reschedule
within 10 business days prior to the start date for a
additional 50% of the training fee already invoiced.
